Used warm, they can help stimulate milk flow before breastfeeding. Warming can be done simply by immersing the thermopad in a bowl of hot water and letting it warm to the optimum temperature.
Used cold, they can help sooth sore or engorged breasts. Cooling the pad can be done simply by placing the thermopad in the refrigerator or freezer.

These pads were a gift from my sister after I had my baby and now I am giving them to all my pregnant friends! They worked great when I was engorged in the first few days of breastfeeding and I keep using them whenever I get a plugged duct: I warm them up and put them on before breastfeeding, have my baby's chin pointing to the area where the plugged ducts (which might require some crazy acrobatics!) and by the end of the feeding the ducts are no longer plugged.

I got these when I was having issues with recurrent plugged ducts, but wish I had gotten them before my baby was born! This is so much easier than using wet facecloths which is what I used when starting breastfeeding - although the facecloths were effective, all my clothes got wet and I had to keep swapping the cloths out because they cooled very quickly. The gel pads are mess-free!
